Request Data Sharing

1. I commit to maintaining the confidentiality and privacy of the data and to not disclose it except to authorized individuals directly involved. 2. I commit to using the data only for the purpose stated in this form and shall not use it for any other purpose without obtaining prior written approval from the Authority. 3. I commit not to disclose or share any of the data provided under this request with any third party without prior written approval from the Authority. 4. Minimal data may be disclosed only for security or judicial purposes, provided that the Authority is notified accordingly. 5. I will not store the data for purposes other than those stated in this request, and I will ensure that all data is securely deleted once the intended purpose is fulfilled. 6. The data requester shall terminate users' access to the data immediately upon role change or completion of the intended purpose. 7. I commit to verifying the security and reliability of all data sharing methods used. 8. I commit to notifying the Authority immediately and within no more than 72 hours of any security incident or data breach involving the disclosed data. 9. I acknowledge full responsibility for compliance with all National Data Governance and Management Regulations and Standards. 10. I confirm that all information provided in this form is accurate, and I accept full responsibility for any consequences arising from inaccurate information. 11. The Authority will evaluate all requests within 30 working days from the date of receipt, in accordance with the Data Sharing Policy issued by the National Data Management Office.
